2. Medical Services

HM Care Clinic provides medical services including but not limited to medically supervised weight loss programs, family practice, alcohol and opioid addiction (Suboxone program), men's health optimization, and aesthetic treatments (Botox, fillers, IPL/laser). All medical services are provided under the supervision of licensed healthcare professionals.

Our weight loss programs utilize FDA-approved medications including Semaglutide and Tirzepatide. Individual results may vary. These medications are prescribed based on a medical evaluation and are not guaranteed to produce specific outcomes. All treatment plans are personalized and subject to physician approval.

3. Patient Responsibilities

As a patient of HM Care Clinic, you agree to:

• Provide accurate and complete medical history and personal information
• Follow prescribed treatment plans and attend scheduled appointments
• Inform your provider of any changes in your health condition or medications
• Comply with all drug monitoring and testing requirements for addiction treatment programs
• Make timely payments for services rendered

4. Appointment & Cancellation Policy

We require at least 24 hours notice for appointment cancellations or rescheduling. Repeated no-shows or late cancellations may result in a cancellation fee or removal from our programs. Same-week appointments are available based on provider availability.

5. Payment & Insurance

Payment is due at the time of service unless prior arrangements have been made. We accept most major insurance plans for applicable services. Weight loss medication programs, aesthetic treatments, and certain specialized services may not be covered by insurance and require out-of-pocket payment. Program pricing is subject to change with reasonable notice.

6. Telemedicine Services

HM Care Clinic offers telemedicine consultations for eligible patients. Telemedicine services are subject to state licensing requirements and may not be available in all states. By using our telemedicine services, you consent to receiving medical care via electronic communication and understand that telemedicine has limitations compared to in-person visits.

7. Confidentiality

All patient information is treated as confidential in accordance with HIPAA regulations and applicable state laws. Our addiction treatment program (Suboxone program) adheres to additional federal confidentiality protections under 42 CFR Part 2. We will not disclose your treatment information without your written consent except as required by law.
